Home Care Package Manager

 We are seeking a professional, passionate, and customer-centred Registered Nurse to oversee the promotion and implementation of Home Care Packages to current or new Burnie Brae clients! This is a great opportunity for an RN who has worked as a community Nurse and is looking to take the next step in their career. This is your chance to get in at the ground level as the Home Care Packages Program revolutionises the aged-care and disability sector.

You will primarily work from our office located at Chermside, but you will be required to travel locally to promote and provide clinical services and health assessments. Your role will be extremely varied and you will thrive in this position if you are an experienced Nurse that also displays strategic thinking, adaptability, and the ability to influence and collaborate with a range of stakeholders.

As the Home Care Package Manager, your role will include the promotion of home care package services, provision of clinical services, supervision of other internal or external RN’s as well as ensuring governance and compliance requirements are met. On a daily basis, you will find yourself…

  • Providing and overseeing clinical services, case management and health checks
  • Developing and implementing training to support the provision of clinical services
  • Identifying and implementing strategies to promote home care packages
  • Participating in networking and collaboration opportunities within the community
  • Ensuring reporting requirements to the Department of Health & Medicare are fulfilled
  • Conducting client reviews and assessments
  • Preparing budgets and care plans in consultations with your clients

In addition to the attributes mentioned above, to be considered for this role you will need:

  • Qualifications in Nursing, with current Registration with the Nursing and Midwifery Board of Australia
  • Experience providing clinical services as a Community Nurse
  • Demonstrable knowledge of the Aged Care Act and Principles and exposure to conducting risk assessments
  • Ability to develop, track and understand budgets and financial information
  • Strong business acumen with a solution-orientated approach to sales
  • Exceptional attention to detail with an empathetic and friendly demeanour
  • Qualifications in Case Management will be highly regarded

Community & Wellness Support Officer

We are seeking a caring, passionate, and open-minded Support Officer to join the Community and Wellness team at Burnie Brae. You will primarily work from our office located at Chermside, as well as be required to travel locally to provide services from time to time. As our clients are primarily from the elderly or disabled community, you will require a relaxed, calm, balanced and accepting demeanour to be successful in this role.

Your role will be extremely varied and will suit someone who is adaptable and flexible to reactive and challenging situations. Your role will include:

  • Providing social support, intervention, and advocacy services to Burnie Brae clients and members
  • Performing intake, referrals, and assessments to link clients and members with services
  • Conducting home visits and welfare checks for clients and members
  • Facilitating and assisting with Burnie Brae community programs
  • Conducting RAS home support assessments
  • Providing support and direction to Burnie Brae volunteers
  • Assisting with the development of policies and KPI reporting
  • Liaising and collaborating with Government and community groups
  • Contributing to the development and promotion of future initiatives

In addition to the attributes mentioned above, to be considered for this role you will need:

  • Qualifications in Human Services, Aged Care, Disability Services, or a related field
  • Experience working with clients in the aged care, disability, and mental health sectors
  • Experience planning and facilitating community programs
  • Ability to manage challenging behaviours and diffuse undesired situations
  • Ability to multi-task and prioritise work to changing deadlines
  • Previous experience leading and engaging with volunteers
  • Ability to problem solve and make quick decisions
  • Proficiency in computers and range of programs and software

Service Support Officer

We are seeking compassionate, open minded and flexible people to join our team at Burnie Brae. As a Service Support Officer, you will be involved in assisting the elderly and young people with a disability to remain living independently in their own homes, while maintaining a positive quality of life. You will have excellent planning skills, the ability to work well under pressure in a fast paced environment and a proven ability to ensure all resources are utilised in the most effective way to support our desired outcomes. As a Service Support Officer, you will be rostering support services across our range of Care Services, including:

  • Transport, Homecare, Home Maintenance, Day Respite, Members activities and more

  • A comprehensive knowledge of Brisbane North and surrounding areas
  • Demonstrated ability to use systems such as rostering platforms, vehicle tracking software & maintenance programs
  • Administration skills with exceptional attention to detail and accurate data and record keeping
  • C Class Driver’s License (MR License desirable)
  • The ability to obtain and provide a National Criminal History Check
  • An empathetic, friendly and helpful demeanour
  • Experience working with seniors and/or people with a disability will be highly regarded
