We accept referrals from physicians seeking specialised exercise physiology treatments for patients diagnosed with chronic diseases. Our range of community-based outpatient care programs, along with their respective referral pathways, are outlined below.
Group Diabetes Exercise and Education Programs - Team Care Plan Referral (Medicare)

Specific Program Referrals
A D904 referral form for white and gold card holders is required prior to commencing services through the Department of Veteran Affairs. Please note the specific condition for white card holders must be listed on the referral. For more information on any of these programs or to refer patients into these programs please contact the Healthy Connections clinic.
Exercise Physiology can be claimed through aged care funding schemes. If you are receiving services through a Home Care Package or the Commonwealth Home Support Program (CHSP), you may be eligible to receive funding towards Exercise Physiology services. For more information, contact Health Connections on 3624 2185 or speak with your provider/package manager.
