At Burnie Brae, our goal is to help you to remain living independently at home for as long as possible.

 

Our Home Care Packages are delivered in a consumer-directed model, providing choice and flexibility in the way that the care and support is provided. Our Home Care Package Manager will meet with you and your family to discuss the things that matter most to you, to ensure an individual Care Plan can be created to reflect your unique needs. Before meeting with your case manager it can be helpful to think about what your priorities are and what goals you have, so you can discuss exactly what you need out of your Home Care Package.

 

Services may include:

 

  • Personal Care: Help with bathing, dressing, mobility and meal preparation
  • Support Services: Cleaning & transport assistance
  • Social Support
  • Home Maintenance/Modifications 
  • Clinical Care: Nursing & allied health support services

 

Burnie Brae can provide a large number of services internally, these include nursing services, home modifications and social support. Burnie Brae can help keep prices down by reducing the need to outsource services.

I approached Burnie Brae about services for my frail aged mother and she has been receiving personal care and in-home respite services for the past twelve month. Mum's carer has been so kind and caring that all of our concerns have disappeared and I can now leave her knowing that she will be well looked after.

Getting Started

 

To receive a Burnie Brae Home Care Package please take the following steps:

 

  • Arrange an assessment: Contact My Aged Care (1800 200 422) and arrange to be assessed by an Aged Care Assessment Team (ACAT) in your area. ACAT will be able to determine the level of care that will best meet your needs.
  • Income test: Have your income-tested by Centrelink in order for your own contribution to be determined.
  • Contact us: Once you’re assessed as eligible you may contact Burnie Brae directly on (07) 3624 2121, to confirm Burnie Brae as your service provider.
  • Plan your support: Our Home Care Package Manager will meet with you and your family to discuss the things that matter most to you, to ensure an individual Care Plan can be created to reflect your unique needs.
  • Start your services: It’s time to get started! We annually assess your needs and make adjustments to your care plan and individualised budget if required.

 

Burnie Brae Home Care Package Fees

 

To ensure prices are kept down and you are receiving the best value, Burnie Brae allows case management fees to be paid directly out of your Home Care Package funds, this means no direct payment is required by the client.

Burnie Brae is an approved provider of level one, two, three and four Home Care Packages.
