At Healthy Connections, we provide a range of allied health services tailored to meet diverse needs. Below are details on how to access our services through various referral pathways and funding options.

NDIS Referrals

As a registered NDIS provider, we offer a variety of services, including:

• Individual programs: Available in-clinic or in-home
• Group exercise programs
• Individual and group hydrotherapy programs

Please note that a service agreement is required before we can commence NDIS services.

GP Referrals

We accept referrals from physicians for specialised Exercise Physiology treatments for patients with chronic diseases. Our community-based outpatient care programs and referral pathways include:

Group Diabetes Exercise and Education Programs (Team Care Plan Referral - Medicare):

• For patients with Type 2 diabetes
• Bulk Billed: Includes 1 Health Assessment and 8x group exercise and education sessions

Enhanced Primary Care (EPC) (GP Management Plan Referral - Medicare):

• Medicare subsidised for up to five individual Exercise Physiology consultations

Specific Program Referrals

• Cardiac rehabilitation (Phase 3 & 4)
  
• Neurological/musculoskeletal rehabilitation (individual and group), e.g., stroke
• Falls prevention classes
• Heart failure and pulmonary rehabilitation

DVA Referrals

For white and gold card holders, a D904 referral form is required prior to commencing services through the Department of Veterans Affairs. Ensure the specific condition for white card holders is listed on the referral.

Aged Care Funding

Allied health services can be claimed through aged care funding schemes. If you are receiving services through a Support at Home or the Commonwealth Home Support Program (CHSP), you may be eligible for funding towards Exercise Physiology, Massage Therapy and Physiotherapy services.
