We accept referrals from physicians for specialised Exercise Physiology treatments for patients with chronic diseases. Our community-based outpatient care programs and referral pathways include:

Specific Program Referrals
For white and gold card holders, a D904 referral form is required prior to commencing services through the Department of Veterans Affairs. Ensure the specific condition for white card holders is listed on the referral.
Allied health services can be claimed through aged care funding schemes. If you are receiving services through a Home Care Package or the Commonwealth Home Support Program (CHSP), you may be eligible for funding towards Exercise Physiology, Massage Therapy and Physiotherapy services.
