Through the Australian Government’s Support at Home program, there are pathways designed to provide additional support during these important times - including the Restorative Care Pathway and End-of-Life Pathway.

Restorative Care Pathway: Regaining Your Independence

The Restorative Care Pathway provides short-term, focused support for older people who may benefit from allied health and/or nursing services to regain or maintain their independence.

Maybe you’ve had a fall and feel less confident on your feet. Perhaps an illness or hospital stay has affected your strength and mobility. Or everyday tasks that used to be easy are now taking more effort.

Restorative care is about setting your goals and working towards them.

That might mean building your strength, improving your mobility, becoming more confident around the home or finding new ways to manage everyday activities.

At Burnie Brae, our team can work together - including Support Workers, Clinical Care Partners and Healthy Connections allied health professionals - so you have people around you working towards the same goal.

It’s not about doing more for you. It’s about helping you do more for yourself.

End-of-Life Pathway: Support at Home When It Matters Most

For someone who has been diagnosed with a life expectancy of three months or less and wants to remain at home, the End-of-Life Pathway can provide short-term support through the Support at Home program.

This can be an incredibly important and emotional time for a person and their family. Having practical support around you can make things a little easier.

Burnie Brae can work alongside the person, their family, palliative care team and other health professionals to help coordinate care and provide support at home.

Our Support Workers can provide practical assistance and companionship, while Clinical Care Partners help coordinate services and make sure everyone is working together.

You don’t have to navigate it alone
